General Privacy PolicyThe MMF Systems Web site (the "Web site") is designed to provide you with information about, and access to, the Web based information management system (the "MMF System") provided by MMF Systems, Inc. ("MMF"). This privacy policy explains the limited degree to which we may collect, use and aggregate personal information about you as you use this Web site and the MMF System. MMF respects your personal privacy and is committed to protecting it. To learn how we protect patient information stored with us with the MMF System, please read our Patient Privacy Policy. The MMF System may use cookies to store certain user preferences. These cookies do not contain any personally identifiable information. They merely enhance the convenience of the user by remembering certain user preferences that control the default behavior of the system on that computer. SecurityThis site has security measures in place to protect against the loss, misuse, and alteration of the information under our control. Every action performed by users of the system is automatically logged for audit purposes. MMF has strong internal policies to manage security and information access. MMF personnel do not need to access any client data stored on the system for performing routine maintenance tasks. Access to client data by MMF personnel is limited to the extent necessary to perform business functions. Your Role in Protecting Your PrivacyIf you access MMF through an Internet account maintained by your employer, please note that your employer may monitor your e-mail or Internet communications. Changes in MMF OperationsIn the event that MMF, or any component of MMF's operations, is merged with, or is acquired by, another entity, then any such successor or acquiring entity may access and use the information that you have provided to MMF, which would be necessary for the entity to effectively continue MMF's business. By using this Web site, you consent to any such use of information by such an entity assuming control of MMF operations because of a merger, purchase of MMF assets or liquidation of MMF assets in bankruptcy or insolvency. ("MMF") sets forth the privacy policies of MMF with respect to physicians, hospitals or other healthcare providers ("Providers") who have registered to use MMF's Web based information management system (the "MMF System"), as well as the patients of Providers using the MMF System to submit Patient Questionnaires. The terms of this Patient Information Privacy Policy are in addition to the Terms and Conditions of Use, the MMF Systems Services Agreement or any other executed agreements you may have with MMF with respect to the MMF System. GeneralWe only store personally identifiable patient information that has been provided to us by (i) a Provider or (ii) a patient who has submitted a Patient Questionnaire through the Web site. We will use this information only for purposes of providing the services and functionalities of the MMF System. We will never disclose personally identifiable patient information except as described in this Patient Information Privacy Policy. We protect personally identifiable patient information by limiting access to the information and implementing server security measures. MMF strives to meet or exceed all applicable industry standards and regulatory guidelines regarding the security and confidentiality of individually identifiable health information, such as the security and privacy provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), and we have carefully considered such requirements in the development of our Web site and this Patient Information Privacy Policy. Patient InformationWe only store Patient Information that has been provided to us by (i) a Provider or (ii) a patient who has submitted a Patient Questionnaire through the Web site. Patient Information stored by MMF does not constitute the original medical record of a patient, but may include medical information that is part of a patient's medical record. MMF acts, at the direction of the Provider submitting such information (the "Submitting Provider"), as a custodian of such information received from the Provider and the Provider's patients. Providers who create and use the Patient Information are responsible for any updates and/or corrections to such information. We will use and release Patient Information only in accordance with the instructions of the Submitting Provider, subject to MMF's Terms of Use, Privacy Policy, the MMF Systems Services Agreement and other applicable agreements. Disclosing Patient InformationIn the event that MMF, or any component of MMF's operations, is merged with, or is acquired by, another entity, then any such successor or acquiring entity may become the successor to our obligations with respect to the Patient Information that you have provided to MMF, which would be necessary for the entity to effectively continue MMF's business. By using the MMF System and pursuant to the Terms and Conditions of Use, you consent to such assumption of MMF's obligations with respect to Patient Information by an entity assuming control of MMF's operations because of a merger, purchase of MMF assets or liquidation of MMF in bankruptcy or insolvency. MMF will seek to assure that any successor entity to MMF assumes all of MMF's obligations and responsibilities as a custodian of Patient Information. |
